9:00am - Noon and 1:30pm - 5:00pm
If you would like to make an appointment, please contact our
office by phone or email. We make
every effort to accommodate the patient by making same day appointments,
even for new patients, in most cases.
Cancellation Policy
If you are unable to keep an appointment, we ask that
you kindly provide us with at least 24 hours notice. We ask for
this advance notice so that we can offer this appointment to another
patient. A fee may be charged if a patient does not show up for
an appointment without sufficient notice.

For those who are Medicare patients, we will submit invoices to Medicare for you, but still require payment at time of service.
If we are not a part of your insurance plan, we require payment at time of service. We will then give you an insurance form you can submit to your insurance to be reimbursed by them.
Financial
We will be more than happy to submit all insurance forms for you and help you recover the most from your benefits. We will do everything we can to help you afford the treatment you need and want.
